1. Evaluate patients/residents within 24 hrs of physician referral.
2. Develop effective treatment plans and obtain approval for services from referring physician, and treat patients/residents according to physician’s treatment plan.
3. Communicate regularly with supervisor, other members of the rehabilitation team, and nursing staff on the patient’s treatment plan, progress, and prognosis.
4. Record evaluations, daily treatment notes, weekly progress notes, 14 day progress report to the physician, and discharge summaries per policy and procedure.
5. Instruct patient/resident’s families or nursing staff in maintenance program.
6. Participate in discharge planning.
7. Secure necessary durable medical equipment for patients/residents to facilitate independence in mobility: i.e. orthoses, walkers, canes, elevated toilet seats, and grab bars.
8. Submits logs and all billing information on a timely basis.
9. Reports any problem areas/equipment within each facility to the appropriate supervisor immediately.
10. Comply with the State Licensing Board of California, Title 22, CMS, and California Labor Board Guidelines.
11. May perform other duties as assigned. |
